Generation Grievance Forms: These are the forms to be used to file Generation grievances as per the Generation contract.
Remember, you must attempt to resolve your complaint with your supervisor prior to filling out the grievance form first.  If you are not satisfied with the resolution, start the grievance paperwork.

EED Grievance Form: (32Kb,PDF) This form is to be used to file EED grievances as per the EED contract. Remember, you must attempt to resolve your complaint with your supervisor prior to filling out the grievance form first.  If you are not satisfied with the resolution, start the grievance paperwork.

Call Center Grievence Form: (44Kb, PDF) This form is to be used to file Call Center grievances as per the Call Center contract.
Remember, you must attempt to resolve your complaint with your supervisor prior to filling out the grievance form first.  If you are not satisfied with the resolution, start the grievance paperwork.

NEW MEMBER FORMS

YOU MUST COMPLETELY FILL OUT THE FOLLOWING FORMS:
MEMBERS THAT JOINED AFTER OCTOBER 1, 2007 WILL BE CHARGED A $20 CHARTER FEE

W9 : Tax form
IBEW Form 66 : Dues Deduction Authorization
IBEW Form 107: Union Membership Application

ALL information is required, including your Social Security Number.
